To receive your best results you will have 6 weeks to complete your coaching sessions (3 sessions, 2-weeks apart). You will receive a coaching contract from me requiring your signature prior to the first session (Choose all that apply).*

I understand that I have 6 weeks to complete my coaching sessions. If I do not schedule and complete the sessions within the 6 week period, I will forfeit the un-fulfilled sessions without a refund.

I will check my email for the contract to be signed and returned before the first session. If I fail to return the signed contract prior to first call, I will forfeit the package without a refund.
